Anaesthetic Practice Appendectomy At Second Trimester Of Pregnancy Nursing Essay This is a reflective essay based on my personal experience as a student anaesthetic practitioner which happened during in one of my clinical placements in an acute hospital. This is a case of a 42 year old at her 24 weeks gestational pregnancy who underwent an emergency appendicectomy under a general anaesthesia with difficult intubation. I will refer to patient as Mrs. G in order to keep her confidentiality (NMC Code of Conduct 2008). The Gibbs reflective cycle will be the framework of my reflection (Jasper 2003) which is shown in Appendix A. Mrs. G. had been admitted due to nausea, vomiting, fever, right abdominal pain, and mild elevation of her white blood cell count which are the presenting symptoms of appendicitis. She underwent ultrasonography of abdomen and confirmed an acute appendicitis, thus, an urgent booking for an open appendicectomy was done. The patient proceeded to the operating theatre with little delay to minimise the sequence to perforation. Such occurrences are uncommon, however, seeing that most cases of appendiceal perforation occur earlier to surgical assessment as described by MacMahon (1991). The theatre team gathered to do a preoperative briefing. The Operating Department Practitioner (ODP), anaesthetist, surgeon, obstetrician, neonatologist, midwife, circulating, scrub nurse and myself were present (WHO 2008). After doing all the routine checks of anaesthetic equipments and drugs as advised by the AAGBI 2004, I went to check the patient (AfPP 2007), the informed consent (DOH 2009) then accompanie d her into the anaesthetic room. Markedly, Mrs. G looked dehydrated, due to fever and reduced oral intake, hence, intravenous fluids was initiated, and pulse, blood pressure, ECG, oxygen saturation and urine output was monitored closely. A Foley catheter was inserted in theatres to ensure adequate urine output. Electrolyte deficiency should be corrected before the induction of general anesthesia (Shields Werder 2002). As a health care professional and a student, I felt pity on Mrs. G because of the severe pain she was having, but what I could only give is to support her. Mrs. G and her husband were in the holding bay and while waiting there to be brought inside the anaesthetic room, apparently, they looked very upset. I stayed with them. I remembered that I should always be present especially when the surgeon, anaesthetist and the whole medical staff discuss the diagnosis and prognosis with them. This action will ensure that someone who will have also heard the discussion dialogue will support the patient, family or carer. We should be aware of the need for privacy when giving information and must endeavour, where the physical environment allows using a closed door environment to ensure absolute confidentiality. We also provide appropriate support to relatives by accompanying them and the patient into the anaesthetic room and to keep them regularly updated and should be given the opportunity to sp eak to the medical staff especially for relatives of critically ill patient and unconscious patients. We must listen to the patients and carers. Their input should be actively sought and not rebuffed. According to Wicker ONeil 2006, we are expected to communicate and give information in a caring, sensitive, friendly and considerate manner and should endeavour to create an atmosphere of confidence and trust when communicating with patients and their families. Careful attention to the evaluation of the airway, antacid pre-treatment and all required preparation for a potentially difficult airway was done for Mrs. G in the preoperative period. It is recommended that from 16 weeks gestation patients undergoing general anaesthesia should be given prophylaxis against aspiration pneumonitis. This usually includes a non-particulate antacid such as sodium citrate 30ml and an H2 receptor antagonist e.g. Ranitidine 50mg intravenously (Yentis et al 2007). Whilst positioning Mrs. G on the operating table, a left lateral inclination of between 15 30 degrees was made. Otherwise, a wedge under the right buttock may be used to minimise aortocaval compression as stated by Aitkenhead Smith (1990). Pre oxygenation was given as it is crucial and should be with a firm fitting mask for at least 3 minutes. As said by Pinnock et al (2003), General anaesthesia involves administration of drugs with possible effects on the foetus. Drugs with good safety records du ring pregnancy were used like Thiopentone, an induction agent, Suxamethonium which is a fast-acting muscle relaxant, Phenylephrine, a vasopressor to increase blood pressure and Metaraminol which is a sympathomimetic used to prevent and treat hypotension (Ponte Green 1994). Drugs that might increase uterine tone like Ketamine and beta-blockers should be avoided (BNF 2010). Obstetricians would request perioperatively, prophylactic tocolytic drugs such as Magnesium Sulfate, Terbutaline and Nifedipine, in order to prevent premature labour. Beta-adrenergic agonists such as Terbutaline are commonly used although their efficacy is vague and may affect maternal tachycardia and pulmonary oedema as stated by Campbell Spence (1997). Recent evidence suggests that calcium-channel blockers like Nifedipine may be effective with a better safety report in line with Simpson Popat 2002. In general, the fewer drugs used overall the better. Drugs such as non-steroidal anti-inflammatory drugs or NSAID s given near to labour may cross the placenta thereby averting the ductus arteriosus to close in relation to Moules Ramsay (2008). I explained to Mrs. G that a rapid sequence induction will be done for her safety and this is by applying a gradually increasing pressure on her neck specifically on the cricoids area. This is also known as Sellick manoeuvre. Intubation might be difficult and so adjuncts for a difficult intubation have been prepared. The operating department practitioner stood behind me whilst coaching me. Suction apparatus was available close at hand, laryngoscope and endotracheal tube was prepared, lubricated and a syringe was attached to the cuff. I did a careful cricoid pressure whilst handling the bougie with the endotracheal tube, but she could hardly see the airway hence, the anaesthetist asked for a guedel oral airway and ventilated her. Later she asked for an Airtraq, which is a disposable laryngoscope with a built-in camera for easy view of the airway. Again, it was difficult. Finally, a fibreoptic scope was used until a successful intubation was achieved. Once the airway was secured, ventilation intended to keep the PCO2 in the standard range for her pregnancy. The MAC or minimum alveolar concentration of volatile anaesthetic was reduced. The foetal heart rate was monitored preoperatively and postoperatively but intraoperatively, it has been difficult as the surgery was abdominal. Along with Allman Wilson (2006), the main concerns of the anaesthetist caring for Mrs. G are the physical response relating to the cardiovascular, respiratory, central nervous and gastrointestinal systems. Stoelting Miller 1994 pointed out that physiological and anatomical changes arise in the respiratory system all throughout pregnancy is due to hormonal and mechanical factors. In accordance with Drain (2003), dyspnoea is always common in pregnancy and oedema of the mucosal airway makes the view at laryngoscopy poor. According to Avidan et al (2003), cardiac output is increased roughly by 40%, and as early as 12-13 weeks, the gravid uterus constrict the lower vena cava when the woman rests flat on her back. Obstruction of the aorta may occur and result in diminished placental blood flow. This is very important when positioning a pregnant patient on the operating table as it can lead to profound hypotension. In severe hypotension and foetal bradycardia patient is turned to the left lateral position Yentis et al (2007). Increased in gastric regurgitation and aspiration might happen during induction of general anaesthesia as stated by Davey Ince 2000 therefore, a rapid sequence induction is obligatory when inducing general anaesthesia. Primarily, blood and oxygen flow must be given to both mother and foetus in order for surgery to be safe Gwinnut (2004). Accurate monitoring, recording and interpretation of any baseline measurement and any subsequent physiological measurement that fall outside of normal limits should be observed. Any fall in blood pressure, oxygen saturation, pulse, respiration, and foetal heart rate should cause alarm for the team. Doctors and anaesthetists must be cautious when operating on pregnant women because of the possibility of a preterm labour which should be observed postoperatively. The surgery has finished and went well. Mrs G was extubated awake and on her side to cut the risk of aspiration of gastric contents. She was transferred immediately to the Recovery and her hus band was informed. I think having both myself and Mrs. G and her family a greater understanding of their condition, treatment, care and the risks of treatment will help alleviate their fears and anxieties. The patient will be satisfied as she understands what is being done to her, therefore, there will be improvement in the overall outcome, speed of recovery, health, and patient experience. The whole team has done their part well and everything for the safety of both the mother and foetus. Pregnant women assumed of having appendicitis are often misdiagnosed and undergo unnecessary appendicectomies which result in early delivery or loss of the foetus. After careful reflection, I strongly believe that there is a need to require more precise diagnosis to avoid needless operations and unfavorable fetal loss. One straightforward solution that I can say is the use of advanced imaging tools, such as ultrasound and magnetic resonance imaging in order to increase diagnostic accuracy. Mrs. G was lucky not because of having a genuine inflamed appendix which has not ruptured so far but the diagnosis was accurately and immediately made through an ultrasound. Preterm labour was also prevented because of the necessary precautions that the whole team has contributed. Number of words: 1, 592

Participative Management in India

Participative Management in the Indian context Evolution of participative management in India Pre- Independence era: Participative management in India has taken to its roots quite early in time . This concept was introduced to the Indian soil by a few humanistic textile mill owners who sought the advice of workers' spokesmen so as to solve industrial disputes. After a few such minor incidents, business houses in India responded to the need of industrial democracy. The Tata Iron and Steel company at Jamshedpur was the first official business organization to set up a works committee which comprised of management and trade union representatives.In 1920, when the Government of India started joint committees encouraged the implementation of such an idea in private enterprises as well. So it can be said that it was that period in India when there were manager and worker bodies that maintained co-operation and provided a common platform for problems related to industrial management. The con cept of participative management in the country took shape when the demand for industrial democracy was felt by the tripartite labour organization ( later on named Indian labour conference), trade unions and political leaders in 1940.Post – Independence era: Participative management became a wider phenomenon post independence when the Industrial Disputes Act of 1947 provided for the establishment of a works committee in every industrial unit that employed more than a hundred employees. Along with this, the Industrial Truce Resolution that was adopted by the Tripartite conference in 1947 promoted labour management co-operation and Industrial Policy Resolution of 1948 provided for the workers' involvement in policies of industrial management.Industrial democracy has also been provided for in the constitution of independent India when it referred to all round democratic participation. In 1976, it was amended to the constitution by article 43- A that it would be imperative on the state to consider the participation of workers in industrial managers. Thereby, participative management became a constituent part of Five year plans and Industrial policy resolutions. There were several other developments with regards to the topic later on. Tripartite labour conference held in May 1977, recommended that a committee be set up to nable the smooth participation of labour in management issues. This led to the formation of a Tripartite committee on worker's participation in management and following were the recommendations of the committee in brief : †¢Corporate level and shop floor level should have a three tier system of labour participation †¢Supervisors or middle managers should have their representation in different participative forums †¢The scheme should be implemented and reviewed by organizations present both at the centre and state level Later on, the Sachar committee was formed to look into the introduction of workers' participation in the Boa rd of Directors.This committee recommended that every company employing more than 1000 workers should comprise of labour participation in the Board of Directors. The committee also suggested that even if the number of employees is less than 1000 in a corporate organization, it should be allowed for workers to have their representation in the Board of Governors. Operating models for participative management in India Since independence the government of India has promoted the participation of workforce in the management by introducing various models for operation from time to time.Five of such schemes introduced by the government are as follows. 1. Works Committee: Establishment †¢Among the first few steps taken to initiate industrial democracy in the country †¢Entailed in section 3 of the Industrial disputes act, 1947 †¢Equal number of employers and workmen representatives in a works committee for every organization with more than 100 employees †¢For Joint consult ation in industrial units Functions †¢Deal with welfare aspects and administration of welfare funds †¢Collective bargaining not included Decisions are not binding on the management because of the committee's consultative and advisory nature Evaluation †¢In spite of increasing awareness of works committees and compulsions to set them up, their number actually started declining and even for the works committees already present, they were not functional and for those that existed and were functional, they performed the wrong function †¢The reason for the above has been attributed to both intra and inter union rivalries and most importantly the persistency of the members to discuss extraneous issues in committee discussions 2.Joint Management Council Establishment †¢The second model of participative management was structured by the tripartite bodies on labour relations in 1958 †¢Joint management councils were set up on voluntarily in both the public and pri vate sectors if there was a required understanding between the employers and workers Objectives †¢To improve working conditions of employees †¢Improve productivity †¢Encourage contributions and suggestions from employees †¢Look into administration of laws †¢To serve as the middlemen or rather a communicating channel between management and employees Structure Joint management council was composed of a maximum of 12 members of which there was equal employer employee representation Evaluation †¢These models of employee participation hardly served the purpose of fulfilling the gaps in the communication between the management and employees. The reason for the same being the reservations of managers who let in certain junior executives into the council and did not equip them with any decision making powers 3. Workers representation on board This model provides for workers to have an opportunity to participate in the ownership of capital as well as in decision making level where the contribution could be meaningful †¢The government launched this model in 1970, when it made way for two worker directors on the boards of fourteen nationalized banks †¢How this model progressed is not much represented in academic schools of thought but it is known that the scheme made no progress at all †¢Presence of multiple unions and the mistrust between management and unions are the main reasons for the failure of this novel idea . Shop council and Joint council †¢The 20 point program mooted in 1975, issued directives to implement the models of shop councils or Consultative committees at the shop floor level and joint councils at the plant level Shop councils †¢Employees in the shop council were nominated by the management †¢Shop councils existed for a term of two years and met least once in a month †¢Shop councils were generally considered to assist the management with progress in matters related to production and safet y Joint councils Joint councils were established for the whole unit and were valid for a term of two years in which they had to meet every quarter †¢The chairman of the council is the chief executive of the unit and the vice chairman was nominated by the worker members of the council †¢Joint councils dealt with matters regarding optimum production, fixation of productivity norms and efficiency †¢It was in the functions of these councils that the term participative management changed to workers participation to allay the apprehensions of the management 5.Participative management by ownership of shares †¢A direct model which ensures the participation of every individual who owns shares of the company †¢Worker is entitled to receive a fraction of the appreciation in capital Conclusion Based on the above models and their evaluation it can be said that the government has so far not effectively tried its best to bring in the theme of participative management into the system. Whatever models and laws the government supported have been very minimalistic in nature and it is sad but true that even these models are not up to the mark.The following points enumerate the reasons for the above situation and throw light on steps to be taken to improve the situation 1. Tender mindedness: Union recognition should initially become a well established practice so that basic issues of drift between the unions and the management can be sorted out before moving on worker participation. 2. Channel of communication : The spirit with which the models of participative management interact with each other should be more relevant than the structure of councils and committees and politics ssociated with the same so that communication is done to resolve issues and not to raise them. 3. Voluntary councils: Voluntary nature of councils definitely creates a situation where issues are resolved not because they are compelled to be but because of the culture and trust betwe en the parties involved. Compulsion in such scenarios would only give way to antagonistic cooperation. 4. Defective form of participation : Councils should be formed to participate as decision making forms and not as consultative bodies because it is this form of functioning that enables worker participation in its true sense.Most of the models of worker management in India are considered to be those where the primary function is that of consultative bodies. How does this ensure industrial democracy in the true sense of the word ? 5. Inadequate representation of supervisors in the management: Supervisors are a key to the word worker participation because they function closely with the management and with the workers. Considering them unbiased, it would make sense to involve supervisors in decision making processes in worker related issues because his decisions would represent both the sides involved. . Lack of motivation : The current scenario is such that the workers are not fully aware and are not enthusiastic about their vital stakes in the scheme because it is a distant dream. Awareness and motivation are thus the need of the hour for a better picture of industrial democracy. 7. Traditional attitude: In spite of so much of history associated with the term of participative management, the workers still have a traditional attitude which keeps them to the lower rungs and abide by whatever the management dictates in terms of decisions. . Lack of strong trade unions: The scheme can be implemented successfully if the trade unions are stronger and serve a common purpose. Unity among workers is an absolute necessary before they want a representation in the management. 9. Deficient institutionalisation : Attention to detail, trails and careful work are what makes the scheme more institutionalised and practical. The agenda of meetings and other relevant issues to be discussed should be drafted beforehand so that the purpose is met. 10.Lack of an evaluation system : It is essential to review each of these models at right intervals so that the workers are aware if their democracy is at their will. The scenario in a capitalist economy Worker participation in India is so far a formality but not a practice that ensures better workman facilities or justice. There have been attempts to include the practice in private undertakings as well. As detailed above, initial laws pertaining to this phenomenon have been imperative on private sector to provide for industrial democracy.Management practitioners like Shiv Nadar of HCL technologies have very smoothly been able to transform organizations by following a bottom-up approach. With all this in view, it can be said that participative management in capitalist economies is not distant dream but as we aren't there yet, it is a distant goal. It is imperative for organizations to help economies reach this goal not for the sake of the word bur for the betterment of the workforce.

The Secret of Ella and Micha Chapter 8

Ella I realize I have more issues than I thought. As soon as we turn onto the Back Road, a passion combusts inside me. It only flames hotter when we pull up to The Hitch, an old abandoned restaurant stationed at the end of the road. It's the perfect set up for street racing, with a long straight road tucked between the lofty trees on the mountains. The sky is black, the moon bright, but there are clouds rolling in. I cringe, thinking of the night on the bridge. We'd been racing before I'd gone there. Micha gets a text message right as we brink the end of the road. He pulls the car to the side, maneuvering carefully across the pot holes. He pushes the parking brake in and checks his phone, shutting it off, and looking torn up. â€Å"What's wrong?† I ask. â€Å"You look upset?† â€Å"Nothing's wrong. Everything's great.† He's lying, but how can I press him to tell the truth when I'm a liar too? â€Å"So this is your surprise?† I will my voice to sound disappointed, but it comes out pleased. Micha gives me a sidelong glance. â€Å"Don't smile, pretty girl. It'll ruin your whole I'm-neutral-and-don't-give-a-shit act.† I opt to remain impartial. â€Å"Who are you planning to race tonight?† â€Å"You mean who are we racing?† He smiles alluringly through the dark cab of the car. â€Å"Well, I thought I'd leave that up to you.† In front of the trees is a line of cars with their headlights on and their owners standing near the front. They're a rough crowd, mostly guys except for Shelia, a big girl with arms thicker than my legs. She's the only girl I've ever truly feared. â€Å"Well, there's Mikey.† I rub my forehead with the back of my hand. â€Å"Does he still got that piece of crap 6 cylinder in his Camaro?† â€Å"Yeah, he does.† Micha leans back in the seat, examining me amusedly through the dark. â€Å"You think that's who I should go for?† â€Å"It's the obvious choice.† I don't like where my thoughts are heading, but I can't shut off my basic instinct. I've always been a hanging-out-with-the-guys kind of girl and therefor there is an abundance of knowledge about cars stashed away in my head. Lila is the first girl I've been friends with. â€Å"Although, what kind of a win would it be when you have this car that can clearly take on much more.† â€Å"You think I should take on someone in my own league?† â€Å"If you want the win to mean anything, then yeah.† We look at each other, like magnets begging to get closer. Yet flip one the wrong direction and they will push apart. â€Å"So which one is it, pretty girl?† He drapes an arm over the headrest behind me and his fingers brush my shoulder. â€Å"The underdog or the big dog.† There's a dare in the air, teasing the real me to come out tonight. I want to give in, just for a few hours, and let my inner ropes untie. I want to allow myself to breathe again, but I fear the loss of control – I fear I'll have to feel everything, including my guilt. â€Å"Micha, I think we should go back.† I put my seatbelt back on. â€Å"This isn't my thing anymore.† He presses his lips together firmly. â€Å"Please can we have a night? Just you and I. I really need this right now.† I pick up on his strange vibe and the sorrow in his eyes. â€Å"Okay, what's wrong? You've seemed a little out of it. Was it bad news on that text you got?† He traces the figure eight tattoo on his forearm. â€Å"Do you remember when I got this?† I absentmindedly touch my lower back. â€Å"How could I forget, since I have the same one on my back?† â€Å"Do you remember why we got them?† â€Å"I can't remember anything about that night.† â€Å"Exactly, yet you'll remember it forever. No matter what happens, which is completely ironic.† He lets his finger linger on the tattoo that represents eternity. â€Å"There's something bugging you.† I tug the bottom of my shirt down to cover up my tattoo. â€Å"Do you want to talk about it?† He shakes his head, still focused on the tattoo. â€Å"Nah, I'm good.† To distract him from his thoughts, I point my finger at a smokin' hot 1970 Pontiac GTO, blue with white racing stripes. â€Å"What about Benny? Does he still have the 455?† Micha's eyes are pools of black liquid. â€Å"You think we should take on the big dog?† â€Å"I think you should take on the big dog,† I clarify. â€Å"I'll just watch you kick his ass.† His expression darkens. â€Å"No way. I'm not racing unless you're in the car with me. It's tradition.† A starvation inside me emerges. â€Å"Alright, I'll ride with you, just as long as you do one thing for me?† â€Å"Say it and it's yours,† he says without blinking. My hunger urges me closer to him. I prop my elbows on the console, and my arms are trembling. He doesn't move, frozen like a statue as I put my lips next to his ear. â€Å"Make sure you win,† I breathe and my body arches into him on its own accord, before I sit back in the seat. His face is indecipherable, his breathing fierce, his gaze relentless. â€Å"Okay, then. Let's go win us a race.† We climb out of the car and hike across the dirt road toward the row of cars and their owners. I shield my eyes from the headlights and wrap an arm around myself, knowing these guys are going to give me crap for how I'm dressed. Micha swings his arm around me protectively. â€Å"Relax. I got you baby.† â€Å"Well, what do we have here?† Mikey, the owner of the Camaro, strides up to us. He's got black hair, a kink in his nose, and his thick neck is enclosed with a barb-wire tattoo. â€Å"Is the infamous duo back again to get their asses kicked?† I roll my eyes. â€Å"You beat us once and that was by default due to a flat tire.† His face pinches as he takes in my shirt, tank top, and curled hair. â€Å"What the fuck happened to you?† Chandra, his girlfriend, sputters a laugh. Her dress is so tight that her curves bulge out of it and her stilettoes make her almost the same height as me. â€Å"Holy shit, she like turned into a little princess or something.† Micha squeezes my shoulder, trying to keep me calm. â€Å"So who's up first? Or has no one decided yet.† Mikey eyes Micha's Chevelle and there's a nervous look in his eyes. â€Å"You think you can just walk in here and play the game after sitting out for nearly a year?† I mouth to Micha, a year? Micha shrugs. â€Å"What? You were gone. Why the hell would I want to race?† â€Å"Again, you need to move on without†¦Ã¢â‚¬  I trail off. Mikey will use what I say against Micha, so I have to watch my mouth. â€Å"We want to race Benny.† Mikey's laughter echoes the night. â€Å"You and what army?† I point at Micha's Chevelle parked near the road. â€Å"That army right there.† Mikey shakes his head and shoos us away. â€Å"That thing don't stand a chance against the GTO. Now run along and come back when you got something bigger.† He's testing my control. A lot. â€Å"As opposed to yours?† I retort, getting into Mikey's face. â€Å"Because that thing's all looks and no go.† Micha directs me back by the shoulders and a trace of amusement laces his voice. â€Å"Easy there, tiger. Let's try not to get our asses kicked tonight, okay?† Benny hops off the hood of his car, flicks his cigarette to the ground, and leaves his buddies to join us. â€Å"What's up? Did I hear someone wanted to race me?† Benny's the kind of guy that everyone respects because they're afraid of him. When he was a freshman he got into a fight at school with a senior twice his size and beat him up pretty badly. No one knows what the fight was over or what happened, but it was enough that everyone became cautious of Benny. Mikey points a finger sharply at me. â€Å"Princess right here wants to challenge you to a race in that thing.† Benny's eyes wander to the Chevelle as he cocks his shaven head and crosses his muscular arms. â€Å"Micha, isn't that your car?† Micha pats my back and winks at me. â€Å"Yeah, apparently she's my spokesperson.† Benny deliberates this and then turns to Mikey, who's glaring at me. â€Å"I don't see what the big deal is. I have no problem with Micha racing. In fact, it might be kinda nice to have a challenge for a change.† Benny slaps Mikey on the back kind of hard and then pounds fists with Micha. â€Å"Thanks man,† Micha says with a respective nod. â€Å"Are you and I going to line up first then?† Benny bobs his head up and down, nodding as he stares at the road pensively. â€Å"Yeah man, I think that'd work.† They chat a little bit more about the rules and what not, while Mikey continues to scowl at me like an angry dog. Once they're done talking, Micha and I walk back to the car, while everyone else scatters toward the starting line located right in front of The Hitch. â€Å"So what's your plan?† I ask. â€Å"Because beating him won't be easy.† â€Å"You're my plan.† He opens the passenger door for me. â€Å"With you in the car, there's no way I won't win, otherwise you'll never let me live it down.† Tucking my head into the car, I drop into the seat and then look up at him. â€Å"I won't make your car go any faster.† He grins, slamming the door. â€Å"Sure you will.† He slides across the front of the hood and climbs into the driver's seat. â€Å"You're such a show off,† I remark. He starts up the engine and it thunders to life. â€Å"That's like the pot calling the kettle black.† I slump back in the seat and fold my arms. â€Å"I may have been a lot of things, but I was never a show off.† He hooks a finger under my chin and angles my head toward him. â€Å"Taylor Crepner's graduation party two years ago. You were standing on the roof with a snowboard strapped to your feet, telling everyone you could make the jump. Sensing my transfer of attitude, he revs up the engine and spins the tires, fishtailing the car to the startup line. He shoots me a smug look, cocking an eyebrow. â€Å"Now that's showing off.† Shaking my head, I restrain a grin. Benny lines up the front of his GTO with Micha's Chevelle and his girlfriend struts up between the two cars. She's wearing jeans and a short t-shirt that shows her stomach. I keep silent as Micha shifts the car over and over again, but something inside me awakens from a very deep sleep. Benny pulls ahead and makes a sharp swerve right in front of us. His red tail lights are blinding in the night and his exhaust is puffing out thin clouds of smoke. Micha speeds up, inching the front end toward the rear of the GTO. As we approach the end, Benny pulls farther ahead, but it's not over yet. Micha has a thing for flipping the car around, without decreasing the acceleration. It's scary as hell, but it works every time. Besides with the longer body of Benny's GTO it doesn't have quite the turning power. We reach the end and I should probably be nervous. The road cuts off into a steep, rocky hill and the space to turn around is narrow, but I've never gotten scared, not even now. I guess I can't change what's in my blood. The GTO begins to slant sideways as Benny turns it. Micha veers to the side to get around him and shoots for the open gap between the car and the trees. I grab the handle above my head, the brakes squeal, and I brace my feet up on the dashboard. It's like being on a merry-go-round on crack. Everything spins – the trees, the sky, Micha. For a second, I shut my eyes and it feels like I'm flying. It takes me back to the night on the bridge. She said she could fly. The car straightens out and Micha floors the gas pedal. Like I predicted, Benny is having a harder time lining back up. By the time we're speeding up the road again, he's a small distance behind us. Micha punches the gas and shifts the car into a higher gear. The long front end of the GTO materializes through my window and Micha floors it, shooting me a look that lets me know I can tell him to slow down if I want. I don't. People flee to the side, panicking at our dangerous speed as we rip through the finish line. It isn't clear who the winner is or who's going to be able to get their car to stop in time, before crushing into The Hitch. Brakes shriek and dust swamps the windows. My body is thrown forward with the car's abrupt halt and I smack my head on the dashboard. Micha works to regain control of the wheel and straightens the car as it skids to a stop. Everything settles and the dust slowly clears. Micha and I stare out the windshield, breathing loudly, our eyes as wide as golf balls. The front bumper of the Chevelle is a sliver away from a very large tree. â€Å"Holy shit,† Micha whispers and looks at me, his eyes bulging. â€Å"Are you okay?† I lower my hand from the dashboard, my chest heaving with my breaths. Plain and simple, but I think it's how I've been all along. I just never admitted. I'm no longer in control. As I incline toward Micha, my heart flutters to life in my chest. My eyes shut and my lips brush his, gently tasting him. It feeds my hunger vaguely and I edge back, letting my eyes open. Micha is looking at me, his eyes pools of blue like the deep spots of the ocean hidden from the world. His hand comes up behind my head and he entices my lips back to his. Something snaps inside me, like a rubber band. With one swift movement, and the aid of my own willingness, Micha lifts me over the console and I straddle his lap, looping my arms around his neck. His hands burrow into my thighs and slip under my skirt onto my bare skin. My breath falters at the intimacy of his touch. No one has ever touched me like this before, without me running away. Usually, being this close to someone sends me into a room packed with self-doubt, panic, distrust, and unfamiliarity. My mind is racing and I clutch onto his shoulders needing my control back. Someone bangs on the window. â€Å"Are you two having fun in there?† I jump back and my cheeks start to heat at the sight of Ethan and Lila staring at us through the window. In his black tee and jeans, Ethan blends in with the night, but his dark, insinuating smile glows. Lila's eyes are magnified and her jaw is hanging open. Micha does nothing to help the situation. A lazy grin spreads across his face as he watches me through hooded eyes, looking very pleased with himself. The adrenaline washes out of me and leaves a numb feeling in its place. I climb off his lap and straighten my skirt and hair before getting out of the car.
